Birth & Death Certificates:

The Seneca County General Health District is now able to issue birth certificates for anyone born in the State of Ohio. Death certificates can only be issued for Seneca County only including the cities of Fostoria and Tiffin. Birth and death records are available for the years 1909 to present.  Click here Birth & Death Application to request a certified copy of a birth or death record.

Certified copies of birth and death certificates are $25.00 per copy. Office hours are Monday through Friday, 8:00 a.m. to 4:00 p.m., closed for lunch from noon to 1:00 p.m.

Mail requests accepted. Send to: Vital Statistics, Seneca County General Health District, 71 S. Washington Street, Suite 1102, Tiffin, OH 44883. Include name of child, date of birth, place of birth, father’s name, mother’s name and maiden name; for a death certificate please include the name of decedent and date of death. Send information along with a self-addressed, stamped envelope and a check or money order made out to Seneca County General Health District in the amount of $25.00 each for a certified copy.
