Birth & Death Certificates:

The Seneca County General Health District is able to issue birth certificates for anyone born in the State of Ohio.  Death certificates can only be issued for deaths occurring in Seneca County including all of Seneca County and the cities of Fostoria and Tiffin. Birth and death records are available for the years 1909 to present.  Click here for a  Birth & Death Application and then follow the instructions below for each copy requested.

Counter service hours are Monday through Friday, 8:00 a.m. to 4:00 p.m.  Certified copies of birth and death certificates are $25.00 per copy.  For your convenience, we accept cash, check or for an additional charge we accept credit and debit cards.

Mail requests accepted.  For birth records fill in sections 1, 3 and 4 on the application above or include the name of the child, date of birth, place of birth, father’s full name, mother’s full name and maiden name. For death certificates fill in sections 2, 3 and 4 on the application above or include the name of the decedent, the place of death and the date of death. Send the information or the properly filled in application above along with a self-addressed envelope, and a check or money order payable to the Seneca County General Health District for $25.00 per certified copy to: Seneca County General Health District, (Attn: Vital Statistics), 71 S.
