Mail requests accepted. Send to: Vital Statistics, Seneca County General Health District, 71 S. Washington Street, Suite 1102, Tiffin, OH 44883. Include name of child, date of birth, place of birth, father's name, mother's maiden name, along with a self-addressed, stamped envelope and a check made out to Seneca county Health Department in the amount of $20.00 for a certified copy.
