WIC is the Special Supplemental Food Program for Women, Infants and Children.
To Sign Up:
- You must be income eligible - call for details
- Bring the following with you for your appointment:
- Proof of Income or current Medicaid Card
- Yourself/your children with ID, such as Driver's License, Birth Certificate and/or Social Security Card
- Child's immunization record
- If pregnant, a note signed by your doctor stating you are pregnant and your due date
- Proof of address.
