Death Certificates

Death Certificates for an individual who passed away in Seneca County, including the cities of Tiffin and Fostoria, may be obtained from the Seneca County General Health District.  Death records prior to 1909 are held with the Seneca County Probate Court and are not available from our office

Certificate Fees

The fee for certified copies of birth or death certificates is $25 per copy. Methods of Payment include cash, check, credit card (with additional processing fee) or money order. Due to changes in State law, we can no longer provide uncertified copies of birth or death certificates.

How to Order

You may order copies of certificates by mail, in person or online with a credit card (additional processing fee applies).

In Person: Visit the Vital Statistics Office at the Seneca County General Health District. In most cases your certificate will be processed and issued in about 15 minutes. 

By Mail: Mail the completed Death Record Request Form along with payment for the certificate and a self-addressed stamped envelope to:

Upon receipt of your request, your certificate will be mailed back to you within 2 business days. Please allow 7-10 business days for the return mail. When requesting by mail please include the following information: Name on record, Date of death and the city and county where the death occurred, 

Online: For your convenience, you can process online requests through VitalChek Network, Inc. – an independent company that the Seneca County General Health District has arranged with to provide you this service.

VitalChek may be reached through their web site. An additional fee of $7.00 (plus shipping) is charged by VitalChek for this service. All major credit cards are accepted, including American Express, Discover, MasterCard or Visa.

NOTE: Effective October 15, 2015 the Ohio Revised Code (ORC) has changed requiring people who need social security numbers on the death certificate to provide proof of why s/he is entitled to such information. This applies only to the first five years after the date of death.
