Requesting Immunization Records

In order to streamline the process in which individuals or parents can obtain immunization records, the Seneca County General Health District (SCGHD) has implemented a new Immunization Record Request Policy.

Immunization Record Cost

The cost for an immunization record is $5.00 per copy requested.  Methods of payment accepted include cash or check/money order.  Checks/money orders should be made out to Seneca County General Health District

How to Order

You may request copies of immunization records by mail, or in person

In Person: Visit the Vital Statistics Office at the Seneca County General Health District. In most cases the immunization record will be processed and issued in about 15 minutes. 

By Mail: Mail the completed Immunization Record Request along with payment for each record and a self-addressed stamped envelope to:

Upon receipt of your request, the records will be mailed back to you within 2 business days. Please allow 7-10 business days for the return mail.
